It starts with a toddler walking into a consulting room. Normal gait, normal coordination. But the eyes are different. Glassy, unfocused, drawn toward the light switch or the printer or whatever inanimate object catches them first. The child moves past the doctor in the chair as though the chair were empty.

Dr Sanjiv Nichani OBE, a consultant paediatric intensivist who has spent 30 years at the sharp end of children's health, has been seeing this pattern in his clinic for the last three or four years. When he asks those families what life at home looks like, the answer is consistent: most of the child's waking hours, in front of a screen.

The language that never came

The observation made clinical sense. The language centre of the brain, Dr Nichani explains, can only be activated by words spoken by real people. A parent, a grandparent, a sibling, a friend. A screen, regardless of how colourful and engaging it appears, cannot do it. It is a passive process. The vocabulary never arrives because the stimulus for acquiring it was replaced by something that cannot provide it.

Those children were not failing to develop language because of any innate deficit. They were not developing language because no one had told their parents what was happening. And as Dr Nichani is clear to stress, that is not the parents' fault. "Parenting is very, very difficult," he told George on Best Beginnings. The harm was not obvious. It is only in the last few years that the science has caught up to the point where we can say with confidence what screens are doing to the developing brain.

The Lamborghini without brakes

The neuroscience, when Dr Nichani explains it, has the quality of something you realise you already half-knew. Deep in the human brain sits the limbic system: the emotional and reward circuitry that is common to all mammals. It is ancient, it is powerful, and it is active from birth. It has to be. It is the survival system. It bonds, it seeks reward, it drives the behaviours that kept our species alive.

The part of the brain that regulates all of this, the prefrontal cortex, is where the distinctly human capacities live: moral judgement, self-regulation, proportionate response. But the prefrontal cortex takes 20 to 25 years to fully mature.

"It's like a Lamborghini with a lot of speed," Dr Nichani says. "But the braking system is faulty because it hasn't developed yet."

This is not a design flaw. It is the architecture of survival. The problem is that this architecture, which served human children perfectly well for tens of thousands of years, was not built for smartphones. The addictive algorithms in the apps and platforms that children now encounter from infancy are engineered specifically to target the reward circuitry, and the developing brain has no means to resist them. The Lamborghini has a perfectly tuned engine. The brakes have not been installed.

Who is responsible

Dr Nichani does not distribute blame evenly. He is clear that parents are not culpable: they were not told what screens were doing, and they cannot be expected to have known. He is equally clear about where he believes responsibility does sit.

"In my opinion," he says, "they're obviously rich and morally bankrupt because they know exactly what they're doing to children. They're only interested in the bottom right hand corner. And that is profit, and snaring children."

On the government's decision to ban social media for under-16s, he is supportive, but sharp: "We're not penalising children. We're protecting children." And on what will actually change corporate behaviour, he is blunt. Not public pressure. Not voluntary commitments. Expensive lawsuits.

The government has published its first ever screen time guidance for children under five. Dr Nichani's own infographic recommends 30 minutes daily for toddlers. He thinks the official guidance of up to one hour is too much.

The oldest medicine

The second part of Dr Nichani's argument is the one that offers a way forward. The same neurochemistry that screens disrupt, movement restores. Regular, consistent physical activity works on the prefrontal cortex and the limbic system directly. It normalises dopamine. It rebalances serotonin. It calms the emotional circuitry and strengthens the brain's capacity for regulation.

He puts it with clinical precision: the drugs prescribed for ADHD and depression work by rebalancing dopamine and serotonin. Physical activity does the same thing, through the same mechanism.

The practical implications are not expensive. Walking to school, or parking at a distance and walking the last stretch. Going to the park. A dance in the kitchen. A water fight in the garden. These are not luxury interventions. They are available to almost every family, and the evidence behind them is as strong as the evidence behind the harm that screens are causing.

What a hub does that a screen cannot

This is where the BabyZone model becomes relevant. Dr Nichani has observed it first-hand. In the hubs, there are no smartphones. There is a parent and a child in the same room, making eye contact, moving together, playing. There is the moment of a smile returned, a giggle, a laugh. The very things that the brain was built to need and that screens, however sophisticated, cannot provide.

The vulnerability Dr Nichani describes in the developing brain is not a weakness. It is the hunger for novelty, for reward, for connection that helped the species survive. The problem is not the child, and it is not the parent. The problem is that something was designed to exploit that hunger, and we are only now beginning to name it clearly enough to act.

The antidote was always there. Movement. Play. A human being, fully present.
